10740: Name Conventions for IRS Matching

1040 Individual

I am getting a reject code (SEIC-F1040-501-01, R0000-504-01, R0000-500-01) that says the name doesn't match the IRS database.

Several different rejects can occur in cases where the format or spelling of the last name is different from what the IRS has on file. The IRS builds their database from information provided by the Social Security Administration, so in most cases, the taxpayer's Social Security card will provide the spelling or format that will allow the return to meet IRS database criteria. In some cases, the IRS may have a different format on file (for instance, if there are multiple last names, last names with punctuation, last names with particles at the beginning, etc.). To resolve mismatch rejections, contact the IRS or Social Security.

Social Security has a toll-free number, (800) 772-1213, available 8 a.m. to 7 p.m., Monday through Friday. Please have the Social Security Number available when calling. Many cities have local Social Security offices that also may be contacted for information. The Social Security Website also contains some valuable information.

Note: The IRS collects names, SSNs, and dates of birth and death from the Social Security Administration's databases. Therefore, the SSA can often help resolve issues that involve names, SSNs, and dates of birth and death of taxpayers.
Examples include: IND-158 and IND-116

If a taxpayer has two last names, the IRS may have the second last name listed as the name control rather than the first. You can attempt to remove the 1st last name on screen 1 and re-file or contact the IRS/SSA for more assistance in correcting this reject and EF Message 6101.
