Drake Accounting®: How do I prepare and file the Michigan UIA-1028?
For filing to the Michigan Department of Labor and Economic Opportunity, DAS produces:
- A PDF of Form UIA-1028, Employer's Quarterly Wage/Tax Report, for record-keeping purposes.
- A .txt file for e-file.
Note: Michigan requires all Employer's Wage/Tax Reports to be filed online.

To prepare the UIA-1028:
- Select the Employees or On the Fly menu, then select State Tax & Wage Forms.
- Make sure MI is selected, then select UIA-1028 from the Forms droplist.
- Select the applicable quarter.
- Review the form and make any necessary entries or edits.
- Make sure the e-File checkbox is marked, then select Save/Print.
Save/Print creates a watermarked PDF copy of the form for record-keeping purposes only. If e-File was marked, a .txt file for upload to the Michigan MIWAM website is also created.
To e-file:
- Log in to the MIWAM account.
- Follow the menu options to upload the wage file.
- The file for upload can be found by browsing to the drive where DAS is installed > DrakeAccounting20YY > DAS20YYData (starting in DAS21) > Clients > client code > EFile > MI > MIUIA1028 and selecting the file named MIUIA1028_YYYY-MM-DD_HR_MIN_SEC.txt (where YYYY-MM-DD_HR_MIN_SEC is the date and time of file creation).
