I have completed the 9465 for the Federal Installment Agreement Request, but the CA Form 3567 is also producing. How do I remove this form?
Information entered on the Federal 9465 will automatically produce and flow to California Form 3567. If you do not wish to file this form, it can be suppressed by going to States > CA > Other > 3567 (Installment Agreement Request) and entering a 0 (zero) in the Payment amount field.
The CA installment agreement request is not sent electronically; it must be mailed.
