The IL-1363 Application for Circuit Breaker and Illinois Cares Rx Form

Important notice from the Illinois Department on Aging:

 

NOTICE: Effective July 1, 2012, the Circuit Breaker Property Tax Relief Grant is eliminated due to the lack of funding. Pending applications will be processed for grant eligibility on a first come basis through June 30, 2012. If an application is postmarked on or after July 1, only the License Plate Discount, Seniors Ride Free and Persons with Disabilities Ride Free benefits will continue to be processed.

As of July 1, 2012, Illinois Cares Rx was terminated. That means that Illinois Cares Rx will no longer help pay for prescription drugs and Medicare Part D premiums. It is important that individuals contact their Medicare Part D prescription drug plan to find out what their premium and prescription drug costs will be beginning July 1, 2012.

The IL-1363, Application for Circuit Breaker, Illinois Cares Rx, License Plate Discount, is not an e-fileable tax form and must be mailed in. It is issued by the IL Department on Aging and is an application for IL residents to get prescription and other benefits. Drake includes this form in the software as a service to our clients since most of the information on this form pulls from taxpayer forms.

 

Because the IL-1363 is not part of tax return and is mailed, there is no Error Code associated with the form that prevents a return from being e-filed.
