Overview
Use the 4506-series forms when the taxpayer needs the IRS to provide a copy of a prior return, a transcript, or (for identity theft victims) a copy of a fraudulent return filed using the taxpayer’s information. In Drake Tax, these requests are completed on screen 4506 (Forms 4506 and 4506-F) and screen 456T (Form 4506-T).
The forms serve different purposes:
- Form 4506, Request for Copy of Tax Return – requests a full copy of one or more tax returns (as filed).
- Form 4506-F, Identity Theft Victim’s Request for Copy of Fraudulent Tax Return – requests a copy of a return filed by someone else using the taxpayer’s identifying information.
- Form 4506-T, Request for Transcript of Tax Return – requests a transcript (summary) of prior return information, not a full photocopy of the return.
See the IRS instructions for each form for fees, processing times, and where to mail the completed request.
Screen 4506
Use screen 4506 to complete Form 4506 or Form 4506-F for the taxpayer.
Any entry on screen 4506 causes Form 4506 to generate in View/Print mode. To produce Form 4506-F instead, mark the Generate 4506-F box at the top of screen 4506 before completing the rest of the screen.
Complete a separate request for each taxpayer who needs a copy or transcript-related form filed under their own name and identifying number. If both spouses need Form 4506 or 4506-F, prepare a separate screen for each taxpayer (for example, press Page Down on screen 4506 after finishing the first request).
Screen 456T
Use screen 456T to complete Form 4506-T, Request for Transcript of Tax Return. Data entered on this screen generates Form 4506-T in View/Print mode.
Screen 4506 also includes a link to related 4506-T information; use screen 456T when the taxpayer needs the transcript request form itself produced from the return.
Blank forms
Clean (unfilled) copies of Forms 4506, 4506-F, and 4506-T are available outside the return from Tools > Blank Forms. Use blank forms when the taxpayer needs a paper copy of the form only and you are not preparing the request inside a return.
Printing and filing
Note Forms in the 4506 series cannot be e-filed with the federal return. Print the completed form from View/Print mode, have the taxpayer sign where required, and mail it to the IRS using the address and instructions on the form.
Many taxpayers can order transcripts online without Form 4506-T. For IRS online options, see Get a Transcript at IRS.Gov (KB 12791).
For form-to-screen references, see Drake Tax – Federal 1040: Screen List (KB 20051).
