Identity theft
Screen 1403 allows you to help your client when identity theft affects their tax account. Use it when someone has stolen the taxpayer’s identity, or when the taxpayer believes a theft has occurred and wants the IRS to mark the account to identify questionable activity.
Complete Form 14039, Identity Theft Affidavit, on screen 1403. In Drake Tax, open screen 1403 from the Other Forms tab of the Data Entry Menu.
Screen 1403
Enter the taxpayer’s information on screen 1403 to produce Form 14039 in View/Print mode. The screen includes tabs for additional sections of the affidavit (for example, the 14039-B tab and the Section F tab). Complete the tabs that apply to the taxpayer’s situation.
You can also prepare Form 14039 for a deceased spouse when the facts support filing on the spouse’s behalf.
For field-level guidance, mailing addresses, and fax numbers, open Screen Help on screen 1403.
Submitting Form 14039
Note Form 14039 is not e-fileable. After the taxpayer signs the form, submit Form 14039 to the IRS with clear, legible copies of required documentation. Use either mail or fax, but not both. Sending the form by mail and fax causes a processing delay.
A blank copy of Form 14039 is also available from Tools > Blank Forms.
For form-to-screen references, see Drake Tax – Federal 1040: Screen List (KB 20051). When identity theft is suspected after a duplicate-accept reject, see Drake Tax – 1040 – Reject – Tax Return Already Accepted (KB 10599).
